Az emlősszív kamrai akciós potenciáljának (AP) pontos repolarizációs mechanizmusa mindmáig tisztázatlan. Az egyes ionáramok szerepének megértését nagymértékben megnehezíti, hogy kinetikai sajátságaik meghatározása hagyományos elektrofiziológiai vizsgálatok során állandó feszültségértékeken, gyakran afiziológiás környezetben történik.
A jelen vizsgálat célja az volt, hogy az AP repolarizációjáért felelős késői típusú kálium áram gyors komponensének (IKr) AP alatti kinetikáját meghatározzuk. Az áram mérésére akciós potenciál clamp technikát alkalmaztunk. A méréseket kutya szív bal kamrából enzimatikusan izolált myocardialis sejteken végeztük. Az IKr kinetikai sajátságait különböző ingerlési frekvenciákon (0,2, 1 és 2,5 Hz) határoztuk meg.
Tapasztalataink szerint az IKr az AP teljes hossza alatt jelen volt. Az áram gyorsan kiépült az AP nulladik és első fázisa során (ez az áram korai komponense), majd a tizedik ms-ot követően növekedési üteme jelentősen lelassult. Az AP plató fázisa alatt az áram lassan tovább növekedett, maximális értékét pedig az AP harmadik fázisában érte el (késői komponens). A korai és a késői komponens egymáshoz viszonyított nagysága jelentős frekvenciafüggést mutatott. Nagy ingerlési frekvenciákon (2,5 Hz) a korai komponens nagysága meghaladta a késői komponens maximális értékének 50%-át. Alacsonyabb ingerlési frekvenciákon nagysága csökkent, 0,2 Hz-es ingerlésnél pedig már nem volt megfigyelhető. A késői komponens nagysága nem mutatott frekvenciafüggést.
Megvizsgálva az áram viszonyát az AP időparamétereivel, jó korellációt találtunk az áram és az akciós potenciál repolarizációs paraméterei között, azonban az IKr a teljes repolarizáló áramnak csak közel 50%-át adta.
Eredményeink azt mutatják, hogy az IKr az ingerlési frekvenciától függően már az AP első repolarizációs és plató fázisában is szerepet játszik. Maximális repolarizáló hatását az AP harmadik fázisában éri el, azonban nem egyedül felelős az AP repolarizációjáért.

THE EFFECT OF THE STIMULATING FREQUENCY ON THE KINETICS
OF THE FAST COMPONENT OF THE DELAYED RECTIFIER POTASSIUM CURRENT UNDER THE ACTION POTENTIAL

The exact mechanism of repolarization of the action potential (AP) of the mammalian ventricular cardiac cells is still not understood. Understanding the role of the ionic currents is being hardened by that the determination of the kinetic parameters of them is often measured by conventional electrophysiological technique on permanent voltage values, which is an aphysiological surround.
The aim of this study was to determine the kinetics of the fast component of the delayed rectifier potassium current (IKr) that plays important role in the repolarization of the cardiac action potential. For measuring this current we used AP clamp technique on canine left ventricular myocardiac cells that were obtained from free ventricular wall with enzymatic dispersion. The kinetic properties of the IKr were studied at different stimulating frequencies (0,2, 1 and 2,5 Hz).
According to our observations the IKr was measurable during the whole AP. The current built up quickly during the zero and the 1. phase of the action potential (this is the early component), then after reaching the 10. ms its increasing rate slowed down. During the plateau phase the current increased slowly further and it reached its maximum value during the 3. phase of the AP (late component). The amplitude of the early and late component compared to each other showed great frequency dependence. At high stimulating frequency (2.5 Hz) the amplitude of the early component exceeded the 50% of the late component. At lower stimulating frequency its amplitude decreased and at 0.2 Hz the early component was not observed. The amplitude of the late component did not showed frequency dependence.
Examining the relation of the current and the AP, we found good colleration between the time parameters of the repolarization of the IKr and the AP, however the IKr gave only the half of the entire repolarizing current.
Our results show that the IKr has a major role in the repolarization process and during the plateau phase depending on the stimulating frequency. It reaches its maximum repolarizational effect under the 3. phase of the action potential, but it is not the only current that is responsible for the repolarization of the AP.
